We are looking for experienced individuals to carry out hte following duties: Too support care leavers make a successful transition to adulthood. To contribute to the assessments, formulation, implementation and review of Pathway Plans through direct interventions and full engagement of the young person. To provide assistance and advice to Care Leavers in relation to developing practical independent living skills, holding a tenancy and developing financial capabilities. Provide coaching and motivational support to enable young people to build the confidence and self-esteem to be successful in adulthood. Safeguard and protect Care leavers as they transition to independence through regular contact and ensuring that they are; effectively engaged; in suitable accommodation; and in education training or employment appropriate to their abilities and aspirations. Work collaboratively with colleagues across agencies with the view to co-ordinating the provision of relevant services and taking reasonable steps to ensure the young person makes use of them. To take on duty tasks and be on the duty rota at least three times monthly. Working patterns: Flexible/agile working is promoted however it is the expectation that in person attendance in the office is completed at least three times weekly. Attendance at Team meetings must be in person. Core working hours are 9am - 5pm.
