As reader coder and Summariser you are to support the data quality team in their work and as your knowledge, skills, and experience progress you will undertake development across the varying responsibilities of the team.
You are required to maintain an up-to-date knowledge around QOF, Information Governance, Information Management, IG Toolkit, Data Protection, Caldicott, Freedom of Information legislation and implement these throughout your daily duties.
Working hours
22.5 hours per week - two positions
Position One
Monday 8am-4pm, Tuesday 8am-4pm, Friday 8am-4pm.
Position two
Monday 9am-5pm, Thursday 9am-5pm, Friday 9am-5pm.
Main duties of the job
Responsible for read coding and linking to existing and new problems by marking it as a new episode / ongoing and decide the appropriate place to attach it in the patient record. Responsible for coding complex medical documents and summarising Medical Record Envelopes when received into practice. Management of NHS Mail and daily incoming mail, this includes scanning and attaching all patient‑related documents to patient records and distribution of other post appropriately. Ensure all patient records are maintained appropriately working to Information Governance standards and the provisions of Data Protection. Managing E‑consults, subject access requests, Insurance reports, death certificates and cremation forms. Processing safeguarding documents in line with the Affinity Care safeguarding administration protocol. Dealing with medical record movement and the EDI links and queries from PCSE. Responsible for assigning and dealing with unassigned tasks in a timely manner that have been received from internal and external sources to the appropriate members of the team or individuals in a timely manner. Support the team with Retinal Screening reports running and saving. Support the team on management of PNL and E‑Cards on the Cervical Screening Management System (CSMS). Provide support across Affinity Care when required.

Undertake all mandatory training in accordance with the Mandatory Training Policy.
Attendance at PLT sessions for ongoing development.
To assist line manager with the achievement of the IG Toolkit by completing appropriate training and any additional information required.
To challenge in a constructive way and actively seek new ways of working, constantly looking for improved processes and procedures across whole systems.
To reduce waste in all forms including ensuring best use of staff time and skill mix, as well as the use of consumables/materials/equipment and other resources such as ensuring surgeries run to full capacity.

To actively take part in relevant training and education sessions.

Qualifications
* An understanding, acceptance and adherence to the need for strict confidentiality

* GCSE English Language, Grade C or above (or equivalent)
* Experience using medical systems
* General Practice or NHS experience
* Coding and summarising experience
* Working knowledge of Microsoft Office Applications
