Suffolk County Council – Digital Care Team opportunity within Adult Social Care at West Suffolk House, Bury St Edmunds, Suffolk IP33 3YU. Hybrid working available.
Salary: £34,434 per annum (pro rata for part time)
Hours: 37 hours per week, flexible working options available
Contract: Permanent
Your role
Your opportunity is to join the Digital Care Team as a Senior Independence and Wellbeing Practitioner (Digital Discharge) working at West Suffolk House within Adult Social Care. This role involves using the Cassius Service to support discharge pathways and improve outcomes for patients, families, and staff. You will work closely with social workers, other Independence and Wellbeing Practitioners, and staff across the locality, including health colleagues in hospital and community settings. This role provides hands-on experience with care technology in practice and delivering change in a fast-paced environment.
Your role and responsibilities
Specific tasks include:
* Supporting other professionals to consider care technology as part of discharge planning.
* Supporting care technology assessment.
* Supporting community referrals to prevent hospital admissions, linking in with Integrated Neighbourhood Team Coordinators.
* Managing the referral process (including benefits tracking and recording).
* Supporting communication and awareness raising of Cassius.
* Managing decisions about eligibility.
* Supporting the Alcove Team in prioritising installations. Delivering
* Delivering training to therapy teams – both hospital and community based.
* Shaping and developing the role in line with business needs and changes.
* Working as part of an integrated team with NHS, Social Care and Cassius.
What you will need
* Excellent communication skills.
* The enthusiasm to contribute to a multi-agency approach for responding within urgent timescales.
* Resilience under pressure and the ability to remain positive when challenged.
* Ability to adapt to changes within the role as and when required.
* Excellent knowledge of care technology and Cassius and how this can positively improve quality of life and maintaining adults' independence.
