As an Advanced Clinical Practitioner, you will play a pivotal role in delivering high-quality, patient-centered care across a variety of clinical settings. You will collaborate with multidisciplinary teams and have the autonomy to practice advanced clinical skills to ensure the best outcomes for patients.
Responsibilities
* Deliver evidence-based care to patients, including diagnosing, treating, and managing acute and chronic medical conditions.
* Work collaboratively with physicians, nurses, and other healthcare professionals to implement comprehensive treatment plans.
* Conduct thorough patient assessments, including histories and physical examinations within the scope of an advanced practice provider.
* Prescribe medications and monitor treatment regimens in accordance with healthcare regulations and best practices.
* Provide expert clinical advice, education, and support to patients, families, and caregivers.
* Promote health and well-being through community outreach initiatives.
* Contribute to policy development, clinical audits, and quality improvement projects to enhance service delivery.
* Act as a mentor and role model for junior staff, students, and trainees.
Qualifications
* Registered Nurse (RN) with active nursing license, and accredited Advanced Clinical Practitioner (ACP) qualification.
* Master's degree in Advanced Clinical Practice, Nursing, or related field is required.
* Proven experience working in a clinical capacity, with specific expertise in advanced practice settings.
* Skilled in diagnostic reasoning, clinical decision-making, and patient management.
* Excellent communication and leadership skills to build strong relationships with diverse patients and interdisciplinary teams.
* Ability to work autonomously while maintaining collaborative relationships across an inclusive healthcare team.
* Commitment to continuous professional development and lifelong learning in advanced clinical practice.
* Familiarity with healthcare IT systems and electronic medical records is a plus.
* Passion for fostering inclusivity and equity in healthcare services.
Day-to-Day
* Conduct patient consultations to evaluate and assess health concerns.
* Perform diagnostic tests, interpret findings, and develop personalized treatment plans.
* Provide direct clinical care, including minor procedures, based on patient needs and clinical scope of practice.
* Coordinate follow-up appointments and monitor patient progress over time.
* Engage in daily interdisciplinary team meetings to discuss patient cases and collaborate on care strategies.
* Attend continuous education programs and professional training opportunities to stay updated on evidence-based practices.
* Lead discussions or training sessions on clinical best practices for nursing or healthcare practitioners.
* Advocate for patients' needs while promoting inclusivity, ensuring all individuals receive equitable care and respect regardless of background or identity.
