Job summary
We are looking for anenthusiastic paramedic (digital badge not required) or Nurse Practitioner, who can prescribe and already has their digital badge, who is keen to develop and work within our growingsupportive team. We would prefer full time hours ( hours/week) but wouldconsider part time working if it fits with our clinical needs.
We are willing to train anddevelop the right candidate to advance their clinical skills. The main scope of work will involve working with our dutyteam of GPs, Nurse Practitioners and Health Care Assistants to provide patientcare, including on the day urgent care.There will be scope to expand/continueareas of interest you may have including chronic disease management and followup consultations.The practice is active in clinical research programmes andthere are opportunities for you to be trained and supported in this stimulatingrole should you wish. Our experienced clinical teams will support and mentor you.We have an open, learning environment with regular in-house educationalsessions, regular audit, safeguarding and SEA meetings.

Main duties of the job
1. Job Role/Purpose: The role will provide a specialist Paramedic/Nurse Practitioner resource forhealth care professionals and service users, working in collaboration withother members of the Multidisciplinary Team. To work within the community for the provision of holistic care for individuals. This will include assessment, diagnosis and treatment, todeliver quality patient services and diagnose, treat, refer or signpostpatients/service users with undifferentiated or undiagnosed condition relatingto minor illness or minor injury. The post holder will use advanced clinical skills toprovide education to service users, promoting self-care and empowering them tomake informed choices about their treatment.

Diagnosing and treating patients presenting with minorillness To receive telephone enquiries from patients, undertake anassessment of the presenting concerns, and give advice as appropriate usingagreed protocols and making timely decisions regarding future management. Thiswill include providing self-care advice, referral to another service or a faceto face appointment in form of a home visit. To work with and refer appropriately to other agencies toenable identified patients to be intensively managed in a pro-active way withthe aim of preventing hospital admission, supporting early discharge andreducing GP contact. To proactively case find patients who are very highintensity users of community and primary healthcare and/or are at high risk ofunplanned admission to hospital Ensure clinical practice is safe and effective and remainswithin boundaries of competence, and to acknowledge limitations. Advise patients on general health care and minor ailments, withreferral to other members of the primary and secondary health care team asnecessary. Works from the Clinical Hub and within communities as anautonomous practitioner caring for patients and families. Works as an autonomous practitioner, in accordance with theHealth Professions Council. Ensure that personal and professional clinicalstandards are maintained. To undertake assessment for patients within the communityand those attending the surgery, using diagnostic skills and initiation of investigationswhere appropriate. To formally and informally impart knowledge and skills tocolleagues promoting peer review and best practice within the work environment. Pathological specimens and investigatory procedures Undertake the collection of pathological specimensincluding intravenous blood samples, swabs etc. Perform investigatoryprocedures requested by the GPs. 3. Key Working Relationships Internal- All members of Sid Valley Practice team! External- Third Party suppliers and providers, Communityteams such as our Urgent Care response team, District Nurses, Reablement teams(employed by RD&E Hospital) SWAST, Sidmouth Hospice at Home 4. Administration and professional responsibilities Participate in the administrative and professionalresponsibilities of the duty team. Ensure accurate and legible notes of all consultations andtreatments are recorded in the patients notes. Ensure the clinical computer system is kept up to date,with accurate details recorded and amended. Ensure accurate completion of all necessary documentationassociated with patient health care and registration. Ensure collection and maintenance of statisticalinformation required for regular and ad-hoc reports and audit. Attend and participate in clinical meetings as required. 5. Training and personal development Training requirements will be monitored by yearly appraisaland will be in accordance with MMP requirements. Personal development will beencouraged and supported by MMP. It is the individuals responsibility toremain up to date with recent developments. Participate in the education and training ofstudents/trainees of all disciplines and the introduction of all members of theSid Valley Practice staff where appropriate. Maintain continued education by attendance at courses andstudy days as deemed useful or necessary for professional development, ensuringrequirements are met. If it is necessary to expand the role to include additionalresponsibilities, full training will be given. Partake and maintain Clinical Supervision plans. 6. Meetings It will be necessary to attend and contribute to variouspractice meetings as requested. The only reason for not attending will beannual, study, or sick leave.
