Community Sister/ Charge Nurse/ District Nurse

We are looking for a District Nurse / Community Sister/Charge Nurse to join our multidisciplinary community team. This includes coordinating and delegating individual patient care by working across boundaries within secondary and social care settings. The role is that of a caseload holder, providing clinical case management for patients with complex health needs who are at risk of deterioration which may result in declining quality of life, avoidable hospital admissions, or an unnecessary length of hospital stay.

Main duties of the job

District Nurse / Community Sister / Charge Nurse

Some of your main duties will include (but not exclusive to):

1. In conjunction with other key professionals, you will be accountable for a patient caseload and delegate responsibility to other professionals within the multidisciplinary team.
2. Apply advanced assessment of physical, mental, and social care needs to provide clinical care and health-promoting interventions that help avoid unnecessary hospital admissions.
3. Interpret clinical signs, symptoms, and diagnostic tests to develop treatment plans and/or treatments in conjunction with medical practitioners as appropriate. Provide advanced clinical advice to the wider healthcare team to improve patient outcomes.
4. Enable patients to be as independent as possible, facilitating a range of self-management strategies.
5. Develop and analyze audits, interpret data, and use findings to enhance clinical practice. Please see the attached job description for full details.

Job responsibilities

Some of your key responsibilities will include (but not exclusive to):

* Case manage a designated caseload of patients and manage/delegate the delivery of care by the integrated team.
* Support the team leader and clinical lead in providing, monitoring, and improving the quality of care, including risk management, complying with and contributing to CQC requirements and clinical audit.
* Oversee all clinical areas, facilitating junior staff to lead on specific areas.
* Deputise for the Clinical Nurse Lead as necessary. Assist in recruitment, appraisal, risk assessment, complaints, and performance management processes.
* Provide clinical supervision/peer review, including active clinical leadership and mentorship to junior colleagues.
* Be aware of the Trust's financial position and its implications on service delivery. Manage clinical stock and equipment maintenance according to Trust policy.
* Review role in practice and participate in appraisal to identify development needs and agree personal development plans.
* Act as a practice assessor for staff and students.


Person Specification

* Registered General Nurse
* Post-registration study
* Community Specialist Practice qualification
* Case Management experience
* Minimum 2 years community experience
* 4 years post-registration experience


Skills and Knowledge

* Range of clinical skills suitable to community nursing
* Experience of Case Management

£38,682 to £46,580 per annum, pro-rata.
