Are you passionate about reducing, preventing, and ending homelessness?
Are you keen to support people who are experiencing homelessness and have associated health needs?
Are you keen to work alongside the NHS to support people being discharged from hospital?
We are looking for a Support Worker to:
1. Be responsible for developing and delivering a high-quality programme of support which promotes client choice and control.
2. Address client support needs, enable them to achieve positive life changes, and ultimately empower them to lead more sustainable lifestyles.
3. Work as part of a team to achieve control and quality outcomes.
