Salary: £30,559 - £32,654, plus 10% unsocial hours for qualifying shifts

Hours per week: 37 hours

Main duties of the job

Our service aids those recently discharged from hospital or under the care of the Rapid Response Service, providing short to mid-term homecare support to promote independence.

As a Senior Support Worker, you will monitor, support, and supervise our dedicated Support Workers, ensuring the delivery of high-quality services for our valued customers.

We welcome candidates with NVQ Level 3 or equivalent substantial experience in a related field, as well as those interested in pursuing relevant qualifications.

Job responsibilities

To lead a team of support workers, providing ongoing monitoring, support, and supervision. Ensure services are delivered to high standards, with robust quality assurance, compliance with legal and statutory requirements, and within effective performance and budgetary frameworks.

Specific duties include:

* Creating person-centered and CQC-compliant support plans with the team.
* Attending initial customer visits to introduce the service and explain the support approach.
* Conducting risk assessments, including environmental, medication, and mobility assessments, and managing associated risks.
* Overseeing medication assessments and ensuring proper documentation and liaising with health colleagues regarding medication concerns.
* Collaborating with partners such as health teams, care homes, agencies, and hospitals, understanding discharge processes, and managing relevant information to ensure safety and risk management.
* Supporting safe care delivery in accordance with regulations and standards, including out-of-hours support.
* Maintaining knowledge of CQC regulations and ensuring paperwork compliance for inspections.
* Facilitating effective communication among professionals, customers, and families to reduce duplication and ensure partnership working.
* Responding promptly to referrals and feedback, providing customers with information about the service, and ensuring confidentiality and accurate record-keeping.
* Monitoring customer progress and supporting team performance, including supervision, training, and induction of new staff.
* Participating in safeguarding practices, recognizing signs of abuse, and following protocols to protect customers.
* Representing the Council professionally at all times and responding to emergencies as instructed.
* Challenging practice for service improvement and leading in team development.


Person Specification


Qualifications

* NVQ Level 4 or equivalent, or significant relevant experience.
* Experience supporting adults in a care environment.
* HSE-recognized First Aid at Work qualification or willingness to obtain it.
* Experience in supervising others or demonstrating required skills.
* Knowledge of legislation related to adult safeguarding, the Care Act 2014, and the Health and Social Care Act 2008.
* Experience with moving and handling procedures.
* Excellent interpersonal and communication skills.
* Good organizational skills and the ability to prioritize tasks.
* Proficiency with social care records and software.
* Physical fitness to perform job requirements.
* Fluency in spoken and written English.
* Experience working within a CQC-registered service.
* Ability to work as part of a team and implement instructions.
* Qualification as a trainer is desirable.


Experience

* Relevant practical experience and advanced knowledge of procedures and practices.
* Experience working with clients with complex needs and conducting risk assessments.
* Ability to guide clients and build trust.
* Proven training delivery skills.
