Role Summary
The Axe Valley Dementia team is a small, dynamic, and supportive team based in Axminster. The team visits and supports people registered at Axminster, Seaton, Colyton, and Townsend House Medical Practices with a diagnosis of dementia. We are looking to recruit a Band 5 Registered Nurse who is motivated and excited to work with people living with dementia and their families/carers over 3 days a week.

The Axe Valley Dementia service is part of the wider Multi-Disciplinary team in the area, specialising in assessment, evaluation, and review of ongoing health and care needs of people living with dementia. The team facilitates 4 Cognitive Stimulation Therapy and Meaningful Activity sessions for people living with dementia in the Axe Valley area.
The team's objective is to keep people living with dementia active, independent, and included in society, to prevent unnecessary hospital admissions, expedite safe discharges, reduce carer breakdown, and delay the need for residential/nursing care. The team prides itself on good care and outcomes for each person.
The successful applicant will have opportunities to develop skills in dementia care, with training and support provided. Good communication, observation skills, basic wound care, and organisation are essential. The applicant should have a caseload and work under the supervision of the Community Dementia Matron.
Knowledge of EPIC (My Care) would be advantageous.
Informal visits are welcome to meet team members and view the working environment.
