We are currently recruiting a dedicated and motivated Band 6 Senior Community Nurse to join our community nursing team covering the borough of Woking.

The Community Team visits patients in their own homes as well as care homes. There are excellent opportunities for training and development of both your clinical and leadership skills. The team has experienced Clinical Leads who will ensure you have the necessary support for your personal development. The community nursing service operates over a 7-day week, from 8.00am to 8.00pm, with a variable shift pattern, including weekends and bank holidays.

Individuals will be expected to deliver and maintain a high standard of clinical and professional nursing care and assist in delivering all aspects of individualised patient care.

Depending on the start date, you may be paid on the 24/25 scales with backdated pay processed once the new pay scales are in effect.

Job responsibilities

Responsible for delivering care and managing service provision according to national and local priorities, organisational policies, procedures, and guidelines, and in line with the Nursing and Midwifery Council.

Principal duties include:

* Managing a defined caseload of patients
* Acting as the named District Nurse for designated GP practices and attending all GSF and MDT meetings
* Undertaking comprehensive nursing assessments, including patients with complex needs
* Developing and adjusting care plans in collaboration with patients and other professionals
* Monitoring medication regimes and advising on safe storage and disposal
* Ensuring the proper use and maintenance of equipment, maintaining infection control standards
* Recognising limitations and referring patients to appropriate healthcare professionals
* Maintaining accurate patient records in line with professional standards
* Making autonomous clinical decisions regarding patient care, including referrals and service discharges
* Identifying risks and escalating issues promptly
* Triaging within the District Nurse Referral Centre and promoting technology use
* Line management of District Nursing team members
* Communicating effectively with all involved in patient care, including out-of-hours support
* Building strong relationships with GPs, social services, hospitals, and community partners
* Completing mandatory training and participating in professional development activities
* Supervising and mentoring team members and students
* Participating in clinical audits and implementing improvements
* Contributing to organisational meetings and initiatives
* Acting as a role model for the 6Cs of nursing
* Managing stock and ordering supplies

Person Specification


Qualifications

* Current 1st level registration (Registered Nurse)
* Degree in relevant healthcare or working towards
* Evidence of ongoing professional development
* Clinical Assessor and Supervisor qualifications
* Educated to Degree/Master level
* Physical Assessment Module
* Nurse prescriber (DN V100)


Others

* Current driving licence and access to a car during working hours
* Flexible, reliable, forward-thinking, and innovative
* Ability to work under pressure
* Passionate about District Nursing


Communication

* Excellent interpersonal skills
* Effective written and verbal communication
* Good presentation skills, e.g., teaching and cascading information
* Ability to facilitate team meetings and supervision groups


Experience and Knowledge

* Minimum of 2 years post-registration experience
* Broad clinical skills relevant to District Nursing
* Ability to undertake full assessments of patients with complex needs
* Mentoring, teaching, and assessing experience
* Experience in service improvement and local change implementation
* Knowledge of health and safety/risk management
* Understanding of healthcare policy impacts
* Proficiency with computers (Word, email, etc.)
* Experience in staff management and appraisal
* Willingness to develop specialist nursing areas

Salary: £40,617 to £48,778 per year, including 5% High Cost Area allowance, pro rata.
