Overview

An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our vibrant, forward-thinking practice. The successful candidate will be an integral part of the nursing team and will be responsible for delivering high-quality patient care to our registered practice population. Working alongside our Nurse Manager, three Practice Nurses, and two GPAs, we especially value chronic disease experience in Diabetes Management. This is a permanent position, 20–24 hours per week. The focus of the role is delivering evidence-based care for patients with long-term conditions and providing preventative health care to the practice population.


Clinical Responsibilities

* Diagnostic procedures: venepuncture, blood pressure, height/weight, ECG, and specimen collection.
* Diagnosis, monitoring, management and treatment of long-term conditions, including non-pharmaceutical approaches and management plans in line with national and local policies.
* Advise patients on general health care and promotion of a healthy lifestyle.
* Chronic and post-surgical wound care.
* Administer injections under individualised prescriptions or PGD within training limits.
* Maintain cold chain of vaccines, including safe storage, rotation and disposal.
* Administer immunisations (e.g., influenza, pneumococcal, tetanus, childhood).
* Support women for family planning and cervical cytology consultations.
* Advise, support and administer vaccinations for patients travelling abroad.
* Assist in minor-surgery sessions.
* Apply infection-control measures in accordance with local and national guidelines.


Core Duties

* Work independently and as part of a busy Nursing Department to meet patient needs and service delivery.
* Be an active member of the Nursing Team: attend meetings, training sessions, support colleagues and cover annual leave or sickness flexibly.
* Act as chaperone when required.
* Support development of others to maximise potential.
* Promote the workplace as a learning environment, encouraging staff to learn from each other and from external good practice.
* Maintain a safe environment for the clinical team, patients and co-workers, ensuring all statutory and local regulations (e.g., Health & Safety, COSHH, clinical waste management, infection control) are implemented and followed.
* Ensure data are recorded accurately, promptly and confidentially in patient records, coded onto relevant templates and documentation.
* Maintain and stock examination and treatment rooms with agreed consumables and clinical equipment.
* Be aware of quality standards and targets set by NICE guidelines, NHS contracts, QOF, etc., working to achieve these standards.
* Promote a professional approach in attire and attitude towards patients and staff, both in and out of the Practice.
* Maintain annual registration with the NMC and other professional bodies (as required) and adhere to the Code of Professional Conduct.


Qualifications – Essential

* Minimum of 1 year recent experience working in primary, community, or hospital nursing as part of a multi-disciplinary team.
* Experience in managing long-term conditions (diabetes, asthma, COPD, hypertension).
* Experience in childhood and adult immunisations.
* Wound care and dressings.
* Phlebotomy.
* Knowledge of infection prevention and control procedures.
* Understanding of safeguarding children and vulnerable adults.
* Experience working autonomously within professional scope of practice.


Desirable Qualifications

* Competence in cervical cytology.
* Experience with chronic disease review templates and QOF.
* Experience using primary care clinical systems (e.g., EMIS).
* Registered Nurse with NMC Registration.
* Evidence of continued professional development.
* Post-registration qualification in Primary Care Nursing.
* Care of the Adult with Diabetes qualification.
