Overview
Due to the expansion of our current services, the Community Diabetes Team is seeking an experienced Diabetes Specialist Nurse to join our team. This role supports the Tower Hamlets Community Diabetes Service, based at Mile End Hospital.
Responsibilities
* Provide clinical and educational support to patients with Type 1 and Type 2 diabetes and their carers.
* Deliver structured education programs for Type 1 diabetes through the DAFNE programme and for Type 2 diabetes through the X‑Pert programme.
* Develop specialist skills enabling management plans, treatment advice, and support for adults with diabetes and their carers.
* Manage a clinical caseload and work autonomously.
* Ensure a smooth transition from primary to secondary care in terms of diabetes care.
* Act as a clinical and educational resource for patients and health professionals.
* Collaborate with the dedicated and enthusiastic team of Community Diabetes Nurse Specialists and other healthcare professionals.
Qualifications
* Experience as a Diabetes Specialist Nurse.
* Strong time‑management, organisational, and communication skills.
* Ability to manage a clinical caseload and work independently.
* Passion for advancing expertise in diabetes technology.
* Commitment to providing high‑quality care to people with diabetes.
