We are looking for a compassionate and enthusiastic AcuteIllness Nurse to join our friendly and supportive clinical team.
Our Acute Illness Nurse will play a critical role in themanagement and treatment of patients presenting with minor illnesses and acuteconditions within a general practice setting. This role involves theassessment, diagnosis, and treatment of a wide range of acute illnesses,ensuring timely and effective care for patients. The Acute Illness Nurse worksindependently but is supported by GPs, nurse practitioners, and otherhealthcare professionals to provide high-quality, patient-centred care.

The post-holder will be responsible for assessing,diagnosing and treating patients with acute conditions. You will also provideexpert professional advice to patients, carers and colleagues and ensure themaintenance of clinical excellence.
We operate a full care navigation appointments system, via a range of communication channels includingtelephone consultations, e-consultation, and face-to-faceappointments.
We have 2 local branch sites in Kimberworth and Ridgeway, you may be required to work at these sites as part of a rotation.
Main duties of the job
* You will have the ability to make professional autonomous decisions when consulting with patients with undiagnosed acute illness, and make assessment of their health care needs.
* As an acute illness nurse you will be seeing patients independently in Face-to-Face or telephone appointments.
* Within patient consultations you will use highly-developed nursing knowledge and skills such as physical examination, applied pharmacology and evidence-based prescribing.
* Maintain accurate and timely patient records using clinical IT systems (e.g. SystmOne).
* Work collaboratively within the practice nursing team, GPs, and other healthcare professionals

Job responsibilities
The postholder will provide high quality clinical careacross face to face, telephone, and where appropriate, home visitconsultations. They will assess, diagnose, and manage a broad range of acutepresentations and long-term conditions, offering evidence-based treatmentplans, health promotion advice, and ongoing support for patients with chronic and acute illnesses.
The role includes requesting and interpreting relevantinvestigations, contributing to same day urgent care and triage services, andensuring all clinical encounters are documented accurately andcontemporaneously using SystmOne.
Working as part of a supportivemultidisciplinary team, the clinician will participate in clinical meetings,audits, and service improvement initiatives, while maintaining compliance withNHS, CQC, and professional standards.
Strong clinical assessment and decision-making abilities areessential, alongside excellent communication and interpersonal skills. Theability to work both independently and collaboratively is key, as is theflexibility to travel between sites when required. A compassionate, patientcentred approach is fundamental, with the capacity to adapt communication styleto meet diverse patient needs and to produce clear, concise, and clinicallyrobust documentation, referrals, and safety netting advice.
While not essential, previous experience within a GPsurgery, a prescribing qualification, and training in minor illness or minorinjury would be advantageous. Familiarity with SystmOne and a goodunderstanding of local NHS pathways and community services would furtherstrengthen the candidates ability to deliver safe, effective care within thepractice.

* Ability to identify determinants of health in the local area.
* Knowledge of public health issues in the local area.
* Awareness of local and national health policy
