Job Overview
Brighton & Hove Wellbeing Service is looking for an experienced Primary Care Mental Health Practitioner to provide rapid assessment, care planning, signposting and brief treatment to individuals referred through self‑referral and GP routes.
Responsibilities
* Conduct routine and priority assessments for individuals across the Primary Care sector.
* Develop support plans and provide brief treatment, including group work.
* Liaise closely with GPs, secondary mental health colleagues and other partners.
* Maintain clinical registers and documentation in line with NHS standards.
Qualifications and Experience
* Core profession registration (Social Work, Occupational Therapy, Nursing) required.
* Registered mental health qualification or equivalent.
* Post‑qualifying experience working with a range of mental health problems.
* Experience or demonstrable knowledge of working in community mental health settings.
Essential Criteria
* Excellent clinical assessment and risk assessment skills.
* Strong communication skills and ability to develop relationships with GPs and other stakeholders.
* Flexibility to deliver care face‑to‑face and digitally.
* Autonomous working with support from the team leader and colleagues.
