Primary Care Clinical Pharmacist — Join Meddygfa Glan Cynon Surgery in Mountain Ash, South Wales. This part‑time role (2–3 days per week) offers a negotiable salary (band 7 or 8 based on experience). The post holder will lead medicines optimisation and quality improvement within a multidisciplinary, patient‑facing team while delivering excellent patient care.
Responsibilities
* Conduct clinical medication reviews for patients with multiple long‑term conditions where medicines optimisation is needed, ensuring a patient‑centred approach.
* Participate in multidisciplinary clinical meetings within the practice and Primary Care Cluster, LMC and Prescribing team within CTMUHB.
* Manage repeat prescription requests and queries within your scope of practice, ensuring patients have appropriate monitoring and safety checks in place.
* Action changes to medicines in response to MHRA alerts, product withdrawals, supply shortages, and relevant local or national guidance.
* Maintain accurate and thorough records of all patient interactions, using appropriate clinical templates and coding in line with practice protocols.
* Take a proactive role in clinical audit and quality improvement initiatives, implementing recommendations where appropriate to enhance patient care.
* Provide patients with clear, independent information regarding their medications and any changes to their prescriptions, referring to other healthcare professionals for additional support when necessary.
* Monitor prescribing patterns within the practice against local health board guidance, making recommendations to prescribers or actioning changes within your scope of practice based on your findings.

Job Description
The post holder will work within their clinical competencies as part of a dynamic multi‑disciplinary team, providing expert support in clinical medicines management, the delivery of structured medication reviews, long‑term condition management, and the review and management of DOACs. They will demonstrate strong clinical leadership in medicines optimisation, safety, and quality improvement initiatives, while contributing to the management of key areas such as the Quality and Outcomes Framework and enhanced services. An essential aspect of the role will be the development of robust systems to manage medicines safely during transitions of care, particularly from secondary care settings ensuring a seamless, safe, and effective transfer process. The post holder will also take the lead in implementing safer prescribing practices and sharing innovations and learning across the practice team and wider Primary Care Cluster where appropriate. The successful candidate will be an experienced and proactive Clinical Pharmacist, confident in acting within their professional boundaries and providing mentoring and leadership support to members of the pharmacy team. They must be able to work independently, using their own initiative, whilst also demonstrating excellent collaboration skills, contributing fully to team projects, quality improvement activities, and wider practice initiatives. Holding an independent prescribing qualification or working towards one is desirable for this role. In addition, the post holder will be encouraged to develop a specialist area of clinical focus aligned to the priorities of the practice and local health board, supporting ongoing improvements in patient care and service delivery. Person Specification
Qualifications
* Essential: Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
* Desirable: Independent Prescriber or currently working towards qualification.
Experience
* Essential: Minimum of 2 years experience as a pharmacist. Experience and an awareness of common acute and long‑term conditions that are likely to be seen in general practice. May hold or be working towards an independent prescribing qualification. Recognises priorities when problem‑solving and identifies deviations from normal patterns and is able to refer to seniors or GPs when appropriate. Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct. Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
* Desirable: Understanding of the inner workings of a primary care setting.
