Overview
An exciting opportunity has arisen for two Clinical Pharmacists to join the North Solihull Primary Care Network (full time and part time considered). This role supports GP practices and optimises the use of medicines across six GP Practices serving a patient population of 40,000.
As a Clinical Pharmacist within our PCN, you will work across a group of GP practices, supported by experienced GPs and a multidisciplinary team that includes GPs, nurses, paramedics, physiotherapists, social prescribing and more. You will manage long-term conditions, carry out structured medication reviews, and support safe prescribing. You will also have the opportunity to run pharmacist-led clinics and, where appropriate, manage minor illnesses.
Main duties of the job
* The post holder is a clinical pharmacist, who acts within their professional boundaries, supporting and working in the GP practices within the PCN. They will be supported by a GP lead who will discuss the development of the role and offer mentorship. They will work autonomously and travel between allocated surgeries to ensure equitable access to the service.
* Work as part of a multidisciplinary team, including GPs, nurses, HCAs, physiotherapists, paramedics, social prescribing link workers, Health and Wellbeing Coaches and administrative staff. Bring expertise around medicines and prescribing to assist the team in providing holistic care.
* Take responsibility for chronic disease management within the practice and undertake clinical medication reviews to manage proactively patients with complex polypharmacy (areas of focus may include CVD, COPD, Asthma, Diabetes, Frailty, etc.).

Detailed Job Description And Main Responsibilities
* The post holder is a pharmacist who acts within professional boundaries, supporting and working in the GP practices within the PCN. They will be supported by a GP Lead / Mentor and GP leads who will discuss development and offer mentorship. They will work autonomously and travel between surgeries to ensure equitable access to the service.
* Work as part of a multidisciplinary team including GPs, nurses, HCAs, physiotherapists, paramedics, social prescribing link workers, Health and Wellbeing Coaches and administrative staff. The Pharmacist will provide expertise around medicines and prescribing to assist the team in delivering holistic care.
* Take responsibility for chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, with focus areas such as CVD, COPD, Asthma, Diabetes, Frailty, etc.
* Provide primary support to general practice staff regarding prescription and medication queries, in person or remotely. Support the repeat prescription system, handle acute requests, medicines reconciliation on transfer of care, and systems for safer prescribing. May run minor ailment clinics in surgery as appropriate.
* Provide clinical leadership on medicines optimisation and quality improvement, and manage aspects of the quality and outcomes framework and enhanced services.
* Ensure integration with community and hospital pharmacy to utilise skill mix, improve patient outcomes, and manage workload.
* Role is pivotal to improving quality of care and operational efficiencies; may be supported to develop to become a non-medical prescriber.

Professional Registration
* Essential: Mandatory registration with General Pharmaceutical Council
Desirable criteria
* Membership of the Royal Pharmaceutical Society
* A member of or working towards Faculty membership of the Royal Pharmaceutical Society
Qualifications
Essential criteria
* Masters degree in pharmacy (MPharm) (or equivalent)
Desirable criteria
* Specialist knowledge through postgraduate diploma level or equivalent training/experience
* Independent prescriber or working towards/intent of gaining independent prescribing qualification
