Job title: Support Worker

Salary: £25,185 - £25,989 pro rata (Band 1)

Hours: Various – flexible hours (early/late/split shifts/weekends)

Contract: Permanent

Location: Extra Care locations in East and West Leeds

About you

As a support worker you will bring to the role:

* Supporting people with everyday tasks like getting washed and dressed and maintaining their independence in doing this.
* Finding new ways to enable people to keep doing the activities they want to do.
* Building people’s confidence and supporting them to access community facilities.
* Candidates must demonstrate effective communication skills, both written and verbal, as well as possessing numeracy abilities.


About the role

Based in one of our extra care services, you will provide support within customers’ own homes. You will have excellent communication skills, be a team player, and be motivated and a critical thinker with the ability to understand the needs of your customers.

The extra care service is available to people over 55 who have housing and care needs. The service helps people maintain their independence at home. The team uses a person-centred, strength-based approach to maximise independence and recovery, working with family, friends and carers to offer shared care, information and advice.

Job description

Job purpose To contribute to providing an integrated package of support to customers to enable them to live independent, safe and fulfilling lives in the community using a strengths-based approach.

Responsibilities

* Work as part of a team and contribute to the development of the service.
* Work flexibly across older people accommodation provision.
* Support new staff in understanding their role.
* Contribute to the assessment of new referrals with senior staff to ensure eligibility.
* Identify needs and strengths with individuals through assessment and support planning.
* Contribute to support plans and risk assessments and promote independence in a safe environment, including housing-related support and Care Quality Commission (CQC) requirements.
* Participate in reviews and multi-agency meetings, applying Mental Capacity Act (2005) principles where appropriate.
* Support individuals to develop practical living skills and manage medications as required, with competency reviews every 6 months.
* Provide personal care, assist with outings, lone working as required, and promote recovery and social engagement.
* Support access to local community resources and services, including housing, education, employment, volunteering and faith organisations.
* Handle small amounts of cash in line with LCC Financial Regulations.
* Safeguard customers within a positive risk management framework and report concerns promptly.
* Communicate with relatives, carers, professionals and signpost to appropriate services.
* Promote involvement in service development and advocacy where required.
* Maintain confidentiality in line with policies and GDPR; complete documentation accurately in CIS systems as required.
* Sleep-in duties as required and comply with Council policies, including Health and Safety.
* Promote best practice and equality of opportunity; respond to changes within the role as needed.


Qualifications

Commitment to undertake a Level 2 Adult Care Worker Apprenticeship within 12 months of commencement. Successful candidates may be offered a Level 3 Lead Adult Care Worker Apprenticeship.


Essential requirements

* Ability to communicate complex and sensitive information effectively with stakeholders.
* Ability to work in a supportive, enabling manner and to support customers to achieve their goals.
* Ability to undertake moving and handling tasks.
* Ability to use a person-centred approach and monitor support plans and risk management.
* Ability to support customers to access housing and make choices about their lives.
* Ability to deal with crises and challenging situations; show empathy and patience.
* Ability to work unsupervised and with flexible, team-based approach.
* Ability to work with diverse service users.
* Ability to work flexibly as part of a team.


Knowledge

* Confidentiality and needs of customers; awareness of vulnerable adults.
* Diversity issues and current policies affecting vulnerable adults.
* Literacy and numeracy at Level 1 or equivalent; Care Certificate within 12 weeks of commencement.


Experience

* Experience in health and/or social care settings; experience working in a team and using person-centred approaches.
