Overview

Are you a qualified Pharmacy Technician who is passionate about making a difference to patient care in the community? Responsibilities

The successful candidate will provide medicines management support and education to patients in their homes, including after their discharge from hospital. By assessing patients' ability to manage their medication and supporting them with tailored care plans, the post holder will help maintain people safe and independent in their own homes. Other duties will include liaising with GPs, community pharmacies, and care providers to ensure safe and effective medication use. By collaborating with social care partners, including the delivery of medication training to care workers, the successful candidate will support the integration of health and social care. Working independently but within a supportive team, the role will utilise clinical skills and evidence-based approaches to improve patient outcomes.

Applicants who are not UK or Republic of Ireland nationals will require sponsorship to obtain either a Health and Care Visa or a Skilled Worker Visa unless they already hold permission to work via another route. Prospective applicants should self‑assess their eligibility for sponsorship by visiting the UK Government's Work in the UK guidance. For eligible candidates, the Health and Care Visa offers reduced application costs and exempts payment of the annual Immigration Health Surcharge.

Qualifications

* Pharmacy Technician registered with the GPhC.
* BTEC Pharmacy Technician, NVQ Level 3 in Pharmaceutical Sciences or equivalent.
* Strong knowledge of medicines management across care settings.
* Experience working in multi‑disciplinary teams and delivering patient‑centred care.
* Excellent communication, organisational, and problem‑solving skills.
* Ability to travel across the UHB locality.
* Welsh language requirements: Welsh Level 1 (or readiness towards it).


Welsh Skills Desirable

This post is advertised as Welsh Desirable. This does not mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short‑listing and selecting candidates. This isn’t 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
