We are seeking a compassionate, organised and proactive care navigator to join our friendly and dynamic General Practice Team. As a key member of our frontline team, you'll be the first point of contact for our patients - helping them access the right care, at the right time, with the appropriate professional.
This is more than a receptionist role. You will be trained in care navigation, a vital skill that ensures sage, timely and appropriate care
Main duties of the job
The Care Navigator is the first point of contact for patients, supporting them in accessing the right care, at the right time, in the right place. This non-clinical role combines administrative and signposting responsibilities with a focus on improving patient experience, reducing pressure on clinical staff, and supporting wider care coordination within the practice and across health and social care networks.

Richmond Clinic is a small traditional GP led practice. there are 3 GP Partners, 1 Salaried GP, 2 nurses, 6 care navigators, 2 admin staff, Deputy Practice Manager and Practice Manager. We are situated close to the M4, J25, giving easy access from Cardiff, Bristol and Monmouth.
We are a small close-knit team who enjoy frequent social occasions and have a relaxed working atmosphere
Job responsibilities
Greet patients and handle telephone, online, or in-person enquiries.
Assess patients needs and direct them to the most appropriate team member, service, or external organisation (clinical or non-clinical).
Promote self-care, digital tools (e.g., NHS App), and local wellbeing services.
Handle appointment bookings and cancellations efficiently.
Care Coordination
Support patients with complex needs, including long-term conditions or those at risk of hospital admission, by coordinating care across services.
Liaise with community services, social care, and voluntary sector providers to support holistic care plans.
Assist in tracking follow-ups and ensuring continuity of care.
Administrative Duties
Maintain accurate and timely patient records on clinical systems - EMIS
Process referrals, forms, test results, and other documentation as required.
Team Integration & Service Improvement
Work closely with GPs, nurses, clinical pharmacists, and social prescribing link workers.
Participate in multidisciplinary team meetings.
Support projects to improve patient access and care pathways.
Person Specification
Experience
* Strong communication and interpersonal skills both written and verbal
* Ability to remain calm under pressure and work in a fast-paced environment
* Competent IT skills, including use of MS Office and patient record systems
* Empathy, discretion, and a patient-focused approach
* Ability to work both independently and as part of a team
* Experience working in a healthcare or customer service setting
* Knowledge of local health, care, and voluntary services
* Experience using EMIS
* Understanding of care navigation or signposting frameworks (e.g., Health Education England's competency framework)
Qualifications
* Good level of English and Maths
* Knowledge of health care settings
