Job Overview
We are seeking to recruit a Community Healthcare Support Worker to join our multidisciplinary CMHT. We support working‑age adults (typically 18‑67 years) with severe & complex mental health difficulties. As community staff we visit people in their homes within the North West Cardiff area and, if needed, escort individuals to other locations as part of the role.
Key Responsibilities
* Build supportive therapeutic relationships on a one‑to‑one basis.
* Use enhanced communication skills, empathy, compassion, and de‑escalation techniques.
* Help service users practice taught skills to manage distress and challenging emotions.
* Encourage a healthy daily routine/activity.
* Provide practical support to access appropriate services (e.g. financial advice, housing support, substance abuse services). Assist with budgeting, shopping and public transport if necessary.
* Graduated work to increase confidence, overcome anxiety and access community as part of the care plan overseen by a qualified team member.
* Assist individuals to attend appointments when required.
* Be a second person/chaperone on visits or in clinic appointments when required.
* Track changes in presentation, recognise signs of relapse and encourage compliance with treatment plans during one‑to‑one contacts.
* Maintain clear and accurate documentation and provide feedback to allocated staff.
Qualifications & Experience
Applicants should ideally have experience working with individuals experiencing mental health difficulties or similar complex presentations. Understanding of common mental illnesses, health and social care systems, and the range of third‑sector support available locally is highly beneficial. The role requires knowledge of lone‑working practices, common mental health presentations, and principles of mental health risk assessment and management. Strong communication skills and the ability to remain flexible and responsive to new or changing situations are key requirements.
