Overview
The Advanced Practitioner - Clinical Pharmacist plays a vital role in improving patient care by providing expert medication management, supporting clinical decision-making, developing the service and collaborating with other healthcare professionals to enhance the quality of care within a Primary Care Network. Focusing on medication safety and optimizing therapeutic outcomes, the role supports PCN goals of improving patient care by developing patient-centred care pathways, reducing hospital admissions, and enhancing overall healthcare delivery within the community.
Rugby Health PCN is a GP-led organisation representing 12 GP practice shareholders and covering circa 125,000 patients. The team currently includes 12 Clinical Pharmacists and 6 Pharmacy Technicians. This post is for a second Advanced Practitioner - Clinical Pharmacist, working with the AP-CP Lead as part of the broader Clinical Pharmacy Team.
Responsibilities
* Assess and triage patients, including same-day triage, and provide definitive treatment where appropriate (including prescribing following policy, PGDs, NICE guidelines and local care pathways) or make necessary referrals to other primary care team members.
* Manage undifferentiated or undiagnosed conditions, identify red flags and serious pathology, and take appropriate action.
* Use complex decision making to inform diagnosis, investigations, and management of episodes of care within a broad scope of practice.
* Adopt a personalised and population-centred care approach to enable shared decision making with patients.
* Complete training to provide multi-professional AP clinical practice and CPD supervision across four pillars to other roles within primary care.
* Lead and line manage the team located across practices within the Rugby Health PCN, providing mentorship and guidance to develop in their roles.
* Oversee the PCN Clinical Pharmacy service provided by Clinical Pharmacists and monitor performance.
* Support the Senior Pharmacy Technician and collaborate with the AP Lead to manage the service and provide cover for leave as needed.
Qualifications
Essential
* GPHC Registration as a Clinical Pharmacist
* Masters degree in pharmacy (MPharm) or equivalent
* Specialist knowledge via postgraduate diploma level or equivalent
* Completed CPPE pathway
* Independent prescriber
* Enrol or completed CP Advanced Practice Programme/Portfolio
* Desirable: Member of the Royal Pharmaceutical Society
* Desirable: Fellow of the Royal Pharmaceutical Society
Experience
* Essential: Minimum of 2 years post-qualification experience
* Experience of supervising a clinical team and leadership
* Experience of working with patients in a community setting
* Experience of managing common acute and long-term conditions
* Ability to work as part of a team and autonomously, with excellent written and verbal communication
* Ability to convey complex information clearly to staff, managers and patients
* Ability to plan and review general medicine optimisation issues in long-term conditions
* Ability to work under pressure and meet deadlines; prioritise and problem solve
* Ability to follow policies, maintain confidentiality and act professionally
* Experience of working across agency boundaries and in local partnerships
* In-depth therapeutic knowledge and understanding of evidence-based healthcare
* Understanding of general practice and primary care prescribing concepts
* Excellent interpersonal, influencing and negotiating skills
* IT proficiency with GP systems (e.g., EMIS, AccuRx)
* Desirable: People/line management experience; experience in primary care settings; contract and performance management

* Full Driving Licence and access to a car
