* You will support and manage a care portfolio, including regular client touchpoints and visits, requiring confident communication skills in a client-facing role.
* You will work closely with clients and carers to ensure a stable, well-planned team of carers and keep all parties informed.
* You will conduct ongoing carer supervisions and annual appraisals for carers working with your clients.
* You should be skilled in problem-solving and issue identification to maintain continuity of care.
* All tasks should focus on delivering high-quality care and customer service excellence.
* This role does not require on-call responsibilities or shift coverage in the community, thanks to the dedicated On Call Care Coordinator team.
