Role
Purpose
1. Contribute to the delivery of an effective Adult social care service as part of a social care team, improving outcomes for People and Carers.
2. Carry out high quality holistic assessments, using strengths based, person centred, community focused, and multi-agency interventions to bring about positive change through developing effective relationships and to secure the best outcomes for people and carers.
3. Undertake the legal and statutory duties of the post as required by the Care Act. Have a good awareness and understanding of Mental Capacity Act, liberty protection safeguards, Mental Health Act and any other relevant legislation
4. Support the appropriate allocation of resources required to maintain independence through the use of Personal Budgets and explore all options that are available to people
5. Manages, plans, prioritises and progress cases of an appropriate level of complexity, seeking support and suggesting resolutions for workload difficulties with due regard to service standards, targets and timescales
Duties
6. Assess and evaluate risk in partnership with service users and carers, where required putting in place appropriate interventions, strategies and contingency plans to mitigate risk and to protect vulnerable adults
7. Clear understanding of Mental Capacity and impacts on practice
8. To pro-actively support and work collaboratively with health services to ensure people receive the required health and social care support within the community, are discharged from hospital within agreed timeframes with appropriate support mechanisms in place, prevent hospital admission and pro-actively work with the principles of admission avoidance
9. The post holder will be required to occasionally work outside "normal" office hours as required by their line manager up to 36 hours per week
10. Respond to the needs of the service by being positive, flexible, and empathetic to customers and carer's needs, working in an integrated way supporting both social workers and occupational therapists to promote independence, choice and control
Work with all other managers within the service to ensure sufficient operational management cover is maintained for the overall Homeless Assessment
Must Have's
11. Knowledge of social care practice and legislation (e.g. Care Act, Equality legislation, Mental health Act, safeguarding and making safeguarding personal, GDPR and Data Protection policies
12. Good communication/assessment and written skills
13. Ability to prioritise case load and ability to work in a fast paced team
14. Experience ( preferable but not essential) of using social care database (Liquid Logic)
15. Experience of completing assessments and/or reviews.
16. Car driver preferred
