Job responsibilities
You will:
* Deliver specialist social workassessments and interventions in both inpatient and community settings.
* Provide compassionate, personcentred support to patients, families and carers navigating complex emotional,practical and safeguarding needs.
* Offer expert guidance tocolleagues on social work practice, legislation and safeguarding.
* Work collaboratively withnurses, doctors, counsellors, chaplaincy and other professionals to ensurecoordinated, safe and effective care.
* Build strong relationships withexternal agencies to advocate for individuals rights, wishes and outcomes.
* Contribute to servicedevelopment, quality improvement, education and innovation within the hospice.
Person Specification
Qualifications
* A recognised Social Work qualification (Masters/Degree/DipSW) and registration with Social Work England.
Experience
* Significant post qualifying experience, ideally including work within palliative care, health, adult social care or another complex setting.
* Strong interpersonal, communication and assessment skills, with the ability to work sensitively and collaboratively.
* A sound understanding of relevant legislation, including the Mental Capacity Act, Care Act and safeguarding frameworks.
* Confidence in managing complex information and maintaining accurate electronic records.
* The ability to work effectively within a multidisciplinary team and use initiative to prioritise work.
* A commitment to ongoing professional development in line with Social Work England requirements.
* A committed and compassionate Social Worker who brings both professional expertise and a genuine passion for supporting people.
