An excitingopportunity has arisen in the Renal Department at Sunderland Royal Hospital.
The RenalAnaemia Nurse Practitioner would like to invite applications from enthusiasticmotivated Registered Nurses with an interest in Renal Anaemia.
The Renal AnaemiaService operates across South Tyneside, Sunderland, Durham and Washingtonmanaging patients with chronic kidney disease and associated anaemia betweenthe hours of 8-6pm, using a mixed format of telephone and face to face clinicsand on occasion, home visits.
The successfulcandidate will receive full training through a set of competency basedassessments and in time, none medical prescriber V300 qualification.
Main duties of the job
Applicationsare welcome from Registered Nurses with excellent clinical skills and theability to communicate well on a professional level within the MDT and thedelivery of sensitive and confidential information to other health careprofessionals and service users
The successfulcandidate will be expected to use good IT and word processing skills forpatient notes and clinic information

Work as part of the multidisciplinary team working under the direction of Anaemia Nurse Practitioner/ Renal manager/Matron.
In collaboration with the Anaemia Nurse Practitioner, develop and deliver the anaemia services, providing a Nurse Led Clinical Anaemia Service, which incorporates, the assessment, counselling, education, and monitoring of patients requiring ESAs, HIF inhibitors and /or iron.
Monitor treatment, compliance and advise on modifications to treatment within agreed guidelines. To provide advice, training and support to enable appropriate patients to safely administer ESAs and HIF inhibitors.
Continue to develop advanced knowledge and skills within the specialist sphere of practice.
Continually reappraise the service provided to patients on ESAs HIF inhibitors and iron therapy, acting as a catalyst for change and improvement.
Support with the planning, delivery and evaluation of care pathways for a designated group of patients under the direction of the Anaemia Nurse Practitioner/M anager/Matron.
Support the development of nursing practice within the specialist field through the use of evidence-based practice, in line with agreed national standards and contribute to the body of nursing knowledge.
Work as an autonomous practitioner within their sphere of practice.
Continuously improve clinical standards, in line with clinical governance framework.
Exercise accountability, as set out in the NMC Code.
Contribute to training and education within the specialist area.
Provide specialist clinical advice and support to patients, their families and clinical staff within the Trust on the management and care of patients within the specialist area.
Provide direct access for patients, either face to face or telephone consultation.
Represent the service nationwide in the absence of the Anaemia Nurse Practitioner/M anager/Matron.
Play a key role in ensuring that national and local standards/targets for the specialist area are met.
Manage and lead nurse-led clinics in relation to reviewing patients and play a key role in optimising complex treatment regimes, facilitating hospital discharge and preventing avoidable hospital admissions.
Support and undertake research relevant to the needs of the specialist practice.

Qualifications
Essential
1. Current 1st level NMC registration.
2. Degree in nursing studies or relevant clinical degree or evidence that currently working towards this (UK equivalent).
3. Mentorship certificate/Practice Assessor training or demonstrable equivalent experience.
4. Clinical skills accreditation or willingness to complete.
Desirable
5. Leadership and Management Course.
6. Basic knowledge of financial management.
