The post holder will practice within a multidisciplinary team and provide care to a group of patients, assessing, planning, implementing and evaluating their care needs. They will be responsible for delivering a high standard of holistic nursing care whilst promoting and maintaining health education and independence. The role holder will ensure assessment and implementation or current and anticipatory care needs, ensuring the patients’ needs are met by Neighbourhood services.
