Main duties of the job
Work withinprofessional boundaries to accurately examine and assess patients with anundifferentiated diagnosis, from a physiological, psychological and socialperspective, and plan clinical care accordingly.
Diagnose and manageboth acute and chronic conditions, integrating both drug and non-drug-based treatmentmethods into a management plan.
Seekadvice and support from a senior clinician as appropriate to inform decisionmaking around patient care.
Prioritise healthproblems and intervene appropriately to assist the patient in complex, urgentor emergency situations, including initiation of effective emergency care.
Review medicationfor therapeutic effectiveness appropriate to patient needs, and administermedication in accordance with PGD/ Independent Nurse Prescribing.

Job responsibilities
The Advanced Nurse Practitioner will bethe first point of contact for patients with all types of health problems. He/she will make decisions and applyappropriate nursing interventions to the presenting patient in their own homeor in the surgery. This will includeinitial history taking, clinical assessment, diagnosis, treatment andevaluation of care. He/she will workautonomously but within the Practices and NMC policies, protocols andguidelines, demonstrating safe clinical decision-making, high levels ofcritical thinking and holistic practice. The post-holder will be expected toact independently but will have access at all times to a GP for advice andsupport as necessary.
Key Responsibilities

Ensurefull and accurate documentation in patient notes of clinical history, physicalexamination, diagnosis and treatment plan.

Beaware of and follow all Practice infection prevention guidelines and proceduresrelevant to the work.
It is the responsibility of all staff, whether clinical or non-clinical,to familiarise themselves with and adhere to current policy in relation to theprevention of the spread of infection.
Clinical staff, on entering and leaving clinical areas and between contacts withpatients, must apply alcohol gel to their hands and also wash their handsfrequently with soap and water. Staff are required to communicate any infectionrisks to the Infection Control lead.
Research projects& clinical audits:
Co-operate and participate as required in any researchprojects and clinical audits within the Practice.
