Job Overview

An exciting opportunity has arisen to join our Community Intermediate Care Team in South East Essex. The current vacancy is within the Cumberlege Intermediate Care Centre (CICC), a 22 bedded Intermediate Care Unit that currently provides stroke rehabilitation. We require a skilled and experienced staff member in physical rehabilitation to assess and treat patients with stroke/neurological diagnoses within a multidisciplinary team that includes Occupational Therapists, Physiotherapists, Speech and Language Therapists, Nurses, Healthcare Assistants and Therapy Assistants. Applicants who wish to work part time will be considered.


Key Responsibilities

* Provide occupational therapy assessment, treatment and evaluation for stroke/neurological patients and frailty patients in a community rehabilitation setting.
* Participate in multidisciplinary rehabilitation services and work autonomously to provide evidence‑based, high‑quality patient care personalised to the individual's goals and needs.
* Deliver a compassionate, dignified and respectful service in line with NHS values, raising concerns through appropriate channels.
* Work with the wider Intermediate Care Service covering Cumberlege Intermediate Care Centre, early support stroke rehabilitation, frailty virtual ward, admission avoidance and re‑ablement/unstable fracture management.
* Manage and coordinate a caseload of patients with complex conditions, maintaining accurate records and risk assessments.
* Provide expert leadership, supervision, and appraisal to junior staff.
* Liaise with other professionals (Reablement, Social Services, Acute Hospitals, Integrated Discharge Teams) to ensure smooth care pathways.
* Support strategic service development in cooperation with the Therapy Lead, Intermediate Care Matrons and Head of Intermediate Care.
* Assist in service evaluation and collection of information for commissioning.
* Maintain therapeutic environment, organize diary, meet clinical caseload demands and give supervision to other Occupational Therapy staff.
* Participate in weekly MDT meetings and daily board rounds, making onward referrals as necessary.
* Ensure health and safety compliance, report risks and coordinate with the Service Manager.
* Plan and deliver occupational therapy within defined areas, prioritise clinical needs, manage conflicting priorities, and make appropriate referrals post‑discharge.
* Adhere to the COT Code of Ethics and Professional Conduct.
* Act as a resource for staff, attend case conferences, facilitate multidisciplinary meetings, and maintain partnership working with other services.
* Engage in continuous professional development, support local and national standards of practice, and contribute to audit, research and practice development.
* Provide line management to junior Occupational Therapists and Therapy Assistants, supervise staff, and ensure effective use of resources.
* Support discharge planning, maintain records, and contribute to the Trust’s development plan for Intermediate Care.

Person Specification


Qualifications

* Current HCPC registration.
* Recognised qualification in Occupational Therapy – Degree or equivalent.


Desirable Qualifications

* Relevant mentoring/supervision qualifications or experience.
* Postgraduate study in Stroke, Falls or Frailty.
* Evidence of continual professional development (CPD).


Skills and Experience


Essential

* Previous experience as an occupational therapist in the NHS or similar UK health systems.
* Experience across a range of clinical areas.
* Ability to prioritise, delegate workload and manage own time.
* Excellent verbal, written and interpersonal communication skills.


Desirable

* Experience working in community settings.
* Experience supervising staff or students.
* Experience using SystmOne.
* Experience in multidisciplinary teams.
* Neurological/Stroke assessment and rehabilitation skills.

Essential

* Proven decision‑making and problem‑solving abilities.
* Enthusiastic and able to work autonomously.

Additional Requirements

* Must have the COVID‑vaccination status required for roles within care homes or CQC‑registered settings, as per current regulations.
