What You Will Do:
Conduct strengths-based assessments to support well-being and independence using community resources
Manage initial assessments, including crisis planning and complex decision-making
Provide advice, signposting, and referrals to appropriate teams or services
Identify and carry out further assessments under Sections 9 and 10 of the Care Act 2014
Create and implement support plans, reviews, and risk assessments
Identify when Section 42 Safeguarding Adult Enquiries are required and act accordingly
Prioritise and respond effectively to a fast-paced, information-rich environment
