Senior Community Nurse - Hospital at Home - Oxford

Are you ready to support young people and their families in the community, preventing hospital admissions?

We're seeking an experienced nurse from a community CAMHS, crisis team, or inpatient background. This could be a secondment opportunity for those looking to gain unique experience in an evolving, specialist team.

You'll help shape our innovative Learning Disability and Autism (LDA) Hospital@Home service, supporting young people to remain at home with tailored care packages. This service, developed under the Thames Valley Provider Collaborative, supports young people with moderate to severe learning disabilities and/or significant autism-related functional impairments at risk of placement breakdown or inpatient admission.

Based at the Warneford site in Oxford, the team works to prevent hospital admissions and enable early discharges, collaborating closely with services across the Thames Valley.

We offer a supportive, inclusive environment with bespoke training, CPD, supervision, and wellbeing support. Full-time or part-time roles (minimum 22.5 hours) are available with flexible working options.

A driving license and access to transport are essential for community visits. Adjustments can be considered in line with the Equality Act 2010.


Main duties of the job

As part of the nursing team, you will have a wide range of knowledge across the fields of nursing, providing holistic and person-centred care and support for young people and their families/ carers within a variety of settings.

The main responsibilities of the role include:

* Undertake specialist nursing assessments in collaboration with the service user and their families/carers and presenting back to the wider MDT. This will include clear risk assessments and add to the formulation of presenting needs for the young person andfamily.
* Work in partnership with service users and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community.
* Regularly review care and outcomes, supporting service users to complete clinical outcome measures and monitoring progress and adapting treatment options as required.
* Support and supervise junior members of the team and to act as a mentor/preceptor to student nurses/trainees.

Person Specification


Knowledge requirements

* Experience of working with clients with a learning disability and/or Autism
* Understand the legal responsibilities of the profession
* Have an understanding of CMHTs and interdisciplinary working


Qualifications

* Professional Qualification RNM or RNC or RNLD (Degree or equivalent)
* Post graduate education specifically relating to client group


Further training

* Ability to work well in a team, to maintain high standards of care and be a good role model


Experience

* Ability to at times manage in a stressful/challenging working environment
* Community work with people with mental health needs.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£38,682 to £46,580 a yearpa / pro rata (plus incentive premium of £3,500)
