An exciting opportunity has become available to work within Best Start in Life Portage Quality and Inclusion Practitioners team within the Family Hubs. The position is for 2 FTE (37 hours) per week working Monday to Friday. One will be based in the North locality and one in the South locality.
Responsibilities
* Support Early Years Settings (nurseries, preschools)
o Help staff deliver high-quality inclusive practice
o Advise on adapting activities for children with additional needs
o Model strategies and interventions (e.g. communication tools, visual supports)
* Work Directly with Children & Families
o Carry out holistic assessments of children and family needs
o Monitor children's progress and plan next steps
o Liaise closely with parents, carers, and professionals
* Deliver Portage Home Visits
o Provide structured home-based learning sessions for children with SEND
o Support development through play-based activities
o Help families build confidence in supporting their child
* Early Identification & Intervention
o Spot children not meeting developmental milestones
o Put in place support strategies early
o Use the graduated approach (assess—plan—do—review)
* Multi-Agency Working
o Work with professionals such as:
+ Health visitors
+ SEND teams
+ Speech & language therapists
o Ensure families get joined-up support
* Support Family Hubs & Parenting Programmes
o Help deliver inclusive parenting programmes
o Guide families to the Local Offer and support services
* Safeguarding & Compliance
o Follow safeguarding procedures
o Ensure children and families are kept safe
o Work within policies (EYFS, SEND, GDPR etc.)
