Job summary
An exciting and rewarding opportunity has arisen for a Case Coordinator to join the team in the West Integrated Transfer of Care Hub (iToCH), currently based at Helston Community Hospital. The role involves hot-desking and shadowing staff during your induction at Camborne Health Office and Penwith Health Centre. You will support clinical staff with admission avoidance, arranging community support, managing non-urgent referrals, and facilitating safe and timely discharges from hospital settings.
You will help provide a seamless service for the people we serve, which includes cross-boundary and collaborative working with multidisciplinary teams, Adult Social Care, and other community agencies.

This is a permanent, full-time position working 37.5 hours per week, including weekends and bank holidays on a rota, as the service operates from 8 am to 8 pm.
Main duties of the job
1. The aim is to foster a collaborative approach to progressing community referrals and hospital discharges, ensuring patients receive quality care in a timely manner.
2. The Case Coordinator will address patients' needs based on referrals and coordinate their care efficiently, utilizing all available resources.
3. Obtain information from health and social care professionals, patients, and families to track patient progress and ensure quality care delivery.
4. Maintain communication with team members or referrers regarding intervention timing, equipment orders/deliveries, and ongoing care organization.
5. Liaise with other agencies and departments to ensure effective communication and care delivery.
6. Develop working relationships with community health and social care teams to support patient care.
