This role is eligible for an incentive scheme of £3,500 paid at intervals over an 18-month period. All payments are pro-rata, non-pensionable and subject to a clawback clause. Please note that only employees external to Oxford Health NHS are eligible for this scheme.

As part of our services across Oxfordshire, we are implementing Primary Care Mental Health hubs to support GP practices with Mental Health assessment and support for patients in their communities.
This will be a fantastic development opportunity as you will be part of this new service. It is a rare opportunity to help shape the new keystone mental health teams as an integral part of the clinical team. Working alongside the lead clinician and colleagues in the team.
We have created new and exciting posts within Keystone Mental Health and Wellbeing Hubs. This role will be based within the Keystone Mental Health and Wellbeing Hub in Banbury.
Main duties of the job
The responsibilities of the role include, but are not limited to:
1. Supporting the Clinical Lead in the development and implementation of the service and your role within it.
2. Supervision and leadership for appropriate staff identified
Screen, triage, assess and coordinate referrals
3. Support patients ready to transition to primary care and develop care plans and prepare people for step down.
4. Map community resources and assets that are available for those with SMI within the specific PCN geography.
5. To support collaborative working between the full range of physical health and mental health services

Essential criteria
17. Current professional registration: Registered Mental Health Nurse, Occupational Therapist.
Desirable criteria
18. Post graduate education specifically relating to client group.
Experience
Essential criteria
19. Community experience in mental health services for adults and older age adults with severe / significant mental illness.
Desirable criteria
20. Experience of working in primary and secondary mental health services.
Knowledge
Essential criteria
21. Specialist knowledge of person-centred care and its application.
22. Specialist knowledge of psychosocial interventions and their appropriate use.
23. Understanding of the mental health needs of adults.
Desirable criteria
24. Specialist knowledge relating to MH needs of young adults age 18-25 and to the needs of those diagnosed with, or with presenting traits of, personality disorder.
