An opportunity has arisen to join the End of Life Care team as a Band 3 Healthcare Assistant on a part-time (13 hours/week), permanent basis. The successful candidate would be welcomed into an established team who work closely with members of the wider multidisciplinary team across all the PCN's in Surrey Downs Health and Care. We have a strong focus on staff support and development, encouraging opportunities for further learning. You will be a Healthcare Support Worker in the End of Life Care Team, providing care and support to palliative patients within the community setting. You will provide a seamless service while a complete package of care is put in place. The post holder will undertake a range of delegated tasks. This role will be supported to develop skills and abilities to work with this client group to provide the appropriate level of care. You will have excellent organisational and time management skills and be passionate about delivering holistic care that has the patient at its centre. You will relish developing new ways of working to improve quality and develop a “one team” culture.
