We are seeking to recruit a Community Nurse Assessor within the District Nursing Team.
The post holder will be responsible for assessing, reviewing, and coordinating the care needs of individuals who may be eligible for NHS Continuing Healthcare (CHC) or Funded Nursing Care (FNC). This includes conducting Specialist Nursing Assessments regionally within the service of Swansea Bay District Nursing Service, across a variety of settings including patients’ own homes and care homes.
Main duties of the job
Working collaboratively with the Swansea Bay District Nursing Team, the post holder will ensure care is person-centred, lawful, evidence-based, and meets the individual’s complex needs. All processes must be coordinated by the District Nursing Service and carried out in line with Health Board policies and procedures.
The post requires robust, evidence-based clinical decision-making in line with the Welsh Government’s National Framework for NHS Continuing Healthcare and the Mental Capacity Act. The role also involves dynamic management of the CHC assessment process, including safeguarding responsibilities, interagency collaboration, and multidisciplinary referral processes

Qualifications
Essential criteria
1. RN/Diploma in Health Care Studies
2. Degree level or demonstrable equivalent experience.
3. In depth understanding of WG policies on Continuing NHS Health care/ NHS Funded Registered Nursing care a) Other professional roles b) Multi professional team working c) Have knowledge of HB and CSSIW
Desirable criteria
4. Partnership working;
5. Certificate in supervisory management
6. Teaching certificate
Experience
Essential criteria
7. Significant experience as Caseload Manager
8. Expert in Nursing Assessments
9. I.T. literate.
10. Leadership skills- a) Good communicator b) Flexible c) Good organisational skills
Desirable criteria
11. ECDL Certificate
12. Experience of working with other professionals –statutory, private and independent services
