We are looking for a dedicated and experienced Care Coordinator to join their growing team on a permanent basis.</p><p>This is an excellent opportunity for a motivated individual who is passionate about delivering high‑quality care services. You will play a pivotal role in coordinating care packages, managing care staff, and ensuring that service users receive the very best standard of care. The successful candidate will thrive in a fast‑paced environment and will be confident in managing complex care requirements across the local area.</p><h3>Day‑to‑Day Responsibilities</h3><ul><li>Coordinating and scheduling care visits for a large team of care workers across domiciliary and complex care packages</li><li>Acting as the first point of contact for service users, families, and healthcare professionals regarding care arrangements</li><li>Managing and monitoring care rotas to ensure full coverage and continuity of care at all times</li><li>Conducting care assessments, reviews, and risk assessments for service users in their own homes</li><li>Ensuring all care plans are up to date, accurate, and reflective of each individuals needs and preferences</li><li>Supporting the recruitment, induction, and ongoing supervision of care staff</li><li>Responding efficiently to any changes in care requirements, last‑minute cancellations, or staffing issues</li><li>Maintaining accurate records and documentation in line with CQC regulations and company policies</li><li>Building and maintaining strong relationships with healthcare professionals, local authorities, and commissioning bodies</li><li>Participating in the on‑call rota as required, with additional payment provided for on‑call duties</li><li>Ensuring full compliance with safeguarding policies and procedures at all times</li><li>Supporting the Registered Manager with audits, inspections, and quality assurance activities</li></ul><h3>Qualifications & Skills</h3><ul><li>Previous experience working as a Care Coordinator, Senior Carer, or similar coordinating role within domiciliary or complex care</li><li>Strong knowledge of care planning, risk assessments, and CQC regulatory requirements</li><li>Excellent organisational and time‑management skills with the ability to manage multiple priorities simultaneously</li><li>Outstanding communication and interpersonal skills, both written and verbal</li><li>Ability to work effectively under pressure in a busy, fast‑paced environment</li><li>Proficiency in care management software and scheduling systems</li><li>Compassionate and person‑centred approach to care delivery</li><li>Strong problem‑solving skills and the ability to make confident decisions</li><li>NVQ/QCF Level 3 in Health and Social Care (or equivalent) is desirable</li><li>A full UK driving licence and access to a vehicle is preferred</li><li>Flexibility to participate in the on‑call rota when required</li></ul><h3>Benefits</h3><ul><li>1 Work From Home day per week for improved work‑life balance</li><li>On‑Call payments in addition to base salary</li><li>Supportive and friendly working environment</li><li>Ongoing training and professional development opportunities</li><li>The chance to make a real difference in peoples lives every day</li></ul><p>We are an equal opportunities employer and welcome applications from all suitably qualified individuals regardless of background.</p></div>
