Job overview
Out of Hours 18.00-23.00. Two evenings a week. All candidates must have a clean UK driving license and use of a car for work purposes.
We are transforming Adult Community Services in Croydon. Health and Social care providers and commissioners have formed the Croydon Alliance Board to deliver whole-system transformation and outcomes desired by over 65s. Outcome Based Commissioning is a new approach to funding and delivery to provide well-coordinated care and promote independence in the most appropriate setting. We offer a comprehensive induction, preceptorship, clinical supervision and ongoing professional development opportunities for career progression. Child care facilities are available once recruited. Main duties of the job
MAIN DUTIES AND RESPONSIBILITIES
1. Promote the mental, physical, emotional and social health of individuals, families and groups.
2. Provide a high standard of care to individuals and families.
3. Be aware of the needs of patients and their relatives and have knowledge of statutory and voluntary services.
4. Be an effective member of a Nursing Team working within the community directorate.
5. Be responsible for caseload management and supervision of the nursing team in the absence of the District Nurse/ Caseload Holder.

Education
* Registered General Nurse or
* NMC Register Part 1 – Adult
* Mentorship for Practice course
Knowledge and Understanding
* Awareness of current issues and developments in community care
* Adherence to NMC Code of Conduct
* Knowledge and understanding of the principles and benefits of reflective practice
Experience
* Ability to demonstrate the role of the community staff nurse
* Previous experience of community nursing

* Ability to maintain accurate records
* Knowledge of audit and research principles
