This is a fantastic opportunity to lead, coordinate, and advocate for patients with complex care needs in a supportive and collaborative environment.The Role:
As a Complex Care Nurse Case Manager, you will be pivotal in designing and implementing person-centered care plans, coordinating services from multiple disciplines (GPs, therapists, social workers), overseeing care packages, and driving continuous improvement through data analysis. You'll also play a role in local business development and commissioning.Key Responsibilities:
* Conduct comprehensive patient assessments
* Develop and implement individualized care plans
* Coordinate care across healthcare disciplines
* Oversee care packages, ensuring safety and effectiveness
* Advocate for patients rights and access to resources
* Educate patients and families
* Monitor patient progress and adapt care plans as needed
* Collaborate with interdisciplinary teams
* Analyze data to improve patient outcomes
* Maintain accurate patient records
What We Offer:
* Highly Competitive Salary: £50,000 - £55,000 per annum (Full Time, DOE)

To Qualify:
* NMC PIN - Registered Nurse in Learning Disabilities or Children's Nursing
* Proven experience in complex care
