Are you a dedicated nurse ready to make a meaningful impact in people's lives? We’re seeking motivated and compassionate team players to deliver high-quality, holistic care within the community as part of our dynamic Rochdale Integrated Neighbourhood Teams.
In this role, you’ll take charge of assessing patients' care needs, developing and evaluating personalised care plans, and providing exceptional nursing support—all within a diverse and engaging environment. Your expertise will help patients stay comfortable and cared for in their own homes, with a special focus on palliative and end-of-life care.
What We’re Looking For:
* Strong independent working skills combined with excellent time management and organisation
* Creativity and flexibility to adapt to changing service needs
* Experience in managing complex cases, including wound care, palliative care, and clinical interventions
* Confidence to work autonomously, supporting patients and their families to maintain independence and quality of life
