We now have an exciting opportunity for Urgent Care Practitioner - Prescriber (UCP(P) to join our team.
In this role, you'll be a vital member of our multi-disciplinary Integrated Urgent Care Clinical Assessment Service (IUC CAS) team, based in Segensworth West, Hampshire. Here, you will play a vital role in delivering high-quality, integrated urgent care to our patients. Working independently in an Out of Hours care setting, you'll provide expert remote triage consultations, ensuring safe, informed clinical decisions with compassion and respect.
Key Responsibilities:
* Work independently while taking full accountability for professional actions.
* Evaluate patients of all ages, from infants to adults, presenting with acute, non-acute, or chronic medical conditions.
* Conduct comprehensive patient assessments, including history-taking, physical examinations, and the development of differential diagnoses and treatment plans, in any clinical setting.
* Facilitate patient care pathways by making appropriate referrals to primary, community, secondary, or tertiary care.
* Prescribe medications within the scope of clinical competence.
* Identify and initiate emergency treatment to protect life or patient welfare, escalating to emergency services when necessary.
* Address social and safeguarding concerns promptly and take appropriate safeguarding actions.
* Ensure safe discharge of patients with clear aftercare and follow-up instructions.
* Deliver person-centred care based on a clear understanding of diverse service delivery methods and contribute to the development of innovative service models that benefit the health system.
* Adhere to key performance indicators (KPIs) while prioritising patient safety.
* Uphold the professional standards and codes of conduct as defined by the NMC/HCPC, ensuring accurate and contemporaneous clinical documentation.
Skills and Qualifications:
Essential
* Professional Registration: Registered with the NMC or HCPC.
* Post Registration Education: Completion of a Level 7, approved Physical Examination and History Taking course. Completion of a Minor Illness/Injury qualification or the COP Specialist Paramedic qualification.
* Prescribing: Completion of an approved Lv7 Prescribing module.
* Previous Experience: At least 6 years post qualification with 3 years of experience working in a primary care GP practice or 3 years' experience in an urgent care setting such as an Urgent Treatment Centre or Emergency Department.
* Safeguarding Expertise: Certified in Level 3 safeguarding for adults and children.
* Mandatory Training: Demonstrates proficiency and compliance with all mandatory training requirements outlined in organisational policies.
* Work Authorisation: Eligible to work in the UK.
* Background Check: Successfully clears an enhanced DBS check.
Working Hours: The IUC CAS covering shifts from Friday ( evenings and nights) / Sat/ Sunday – days and nights
Pay: £26.94-£45.88 per hour
