Overview
We are seeking a highly organised and detail-oriented QOF coordinator and administrator to join our team. The ideal candidate will possess strong office experience and administrative skills, ensuring the smooth operation of our daily activities. This role is crucial in supporting various departments and facilitating effective communication within the organisation.
Duties
* Coordinate and manage patient recall systems for long-term condition reviews, ensuring annual reviews are completed in line with QOF and IIF (Investment and Impact Fund) requirements.
* Support the optimisation of clinical processes and pathways to enhance patient care and improve practice performance against QOF and IIF targets.
* Work collaboratively with the Practice Admin Team to monitor and manage capacity and access, utilising AI tools where appropriate to support data-driven decision-making.
* Facilitate seamless communication between patients and multidisciplinary teams to ensure holistic, patient-centred care.
* Contribute to the ongoing review and improvement of recall systems and care coordination processes.
* Assist in monitoring capacity and access digital tools and support demand management strategies across the practice.
* Maintain accurate records and contribute to performance monitoring and reporting requirements.
Requirements
* proven office experience with a strong background in administrative roles
* proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) and Google Workspace applications
* experience of working in a primary care environment
* strong organisational skills with the ability to manage multiple tasks simultaneously
* experience of working with Quality Outcome Framework (QOF) targets
* demonstrated clerical experience with attention to detail in all aspects of work
* effective communication skills, both verbal and written, to interact professionally with colleagues and clients

Pay: £12.21-£12.95 per hour
