The Role
The main role the the Throughcare and Aftercare Nurse is to support the foundations of the Promise and young people in receipt of Throughcare and Aftercare. A primary function of this role would be to provide the scaffolding of help and support to assist care leavers in achieving the best possible outcomes and transition successfully into adulthood. Care will be supported with view to improved assessment of health needs and review of any support pathways and promote self-efficacy. For the young people identified, aged up 25 years (until 26th Birthday). The postholder will work in close partnership with the Throughtcare and Aftercare team and all identified partners and agencies.
The post holder will be responsible and accountable for maintaining all clinical, staff and information governance.

What we'll need you to bring
Registration requirements
* Registered nurse at degree level or equivalent
* Post-registration qualification in public protection / child protection
Qualifications & Knowledge
* Educated to Masters level in a relevant nursing area, or
* Can demonstrate an equivalent level of knowledge gained through, for example, completion of NES modules on Assessment, Leadership, and Critical Thinking.
* Can provide advice using significant understanding and application of legislation relating to information sharing, data storage and public protection
* Knowledge of health and social care pathways related to safeguarding, early intervention and protection
Qualities and Skills
* Can demonstrate the following skills and attributes:
* Experience of caseload management and planning to meet deadlines
* Experience of using a strength based approach working in partnership with families
* Able to use a number of assessment tools used to decide referral pathways
* Management of risks and concerns using critical thinking to enable effective decision making
* Ability to provide professional advice within a multi-agency setting that puts children’s interests and safety first
* Can lead discussions and make decisions based on an assessment of a range of factors in complex situations
* Ability to develop and implement policies, procedures, service standards and improvements
* An ongoing commitment to continuing professional development
* Please note for all vacancies where a driving licence is required this must be a full UK/EU/EEA licence.
It would be great if you also have
* SPQ in related field
* Post-graduate qualification in public protection /child protection
* Experience of working with care experienced children or young people
* Knowledge of quality improvement methodologies
* Can demonstrate understanding of the impact and vision of The Promise and UNCRC
