Support Coordinator Role

As our Support Coordinator, you’ll provide the support needed for customers to develop skills for independent living. Our service offers accommodation to customers aged 16–25 who are homeless or at risk of homelessness.

Typical Day
1. You’ll work with a team of Support Coordinators to deliver high-quality service that meets or exceeds Home Group KPIs and OFSTED standards.
2. Under the supervision of your Senior Client Service Manager and Senior Support Coordinator, ensure customers receive support to develop independence and achieve their goals.
3. Support customers with diverse needs, including mental health issues, housing support, complex needs, and challenging behaviour.
4. Create and update support plans and risk assessments, assisting with daily activities like meal prep, shopping, budgeting, maintaining tenancies, and moving to independent accommodation.
5. Ensure properties are well maintained, clean, and homely to reduce voids and provide a pleasant living environment.
6. Engage in informal chats and collaborate with colleagues without rushing from one customer to the next.

You Have
* Experience creating person-centred support plans, risk assessments, and reviews
* Experience coordinating and assessing customer referrals
* Ability to work independently, stay calm under pressure, and be resilient
* Confidence to challenge behaviours that conflict with our values
* Understanding of our customers’ life stages, passion to advocate for them, and experience in housing or care support

Job Details
1. The service operates 24/7. You will work on a 4-week rolling rota, including weekends and bank holidays, without night shifts.
2. Flexibility may be required to meet customers’ needs. We aim to accommodate your work/life balance requests.
3. Morning and late shifts may include lone working periods.
4. Basic proficiency in Microsoft Word and Excel is essential.
5. You should be comfortable using technology for support plans, online learning, and collaboration.
6. An Enhanced DBS check is required; we will cover the cost.
