Llandaff North Medical Centre is looking for a primary care pharmacist to join our clinical team in a new role for a fixed-term 9-month contract (16 hours over two days per week) with possible extension contingent on external funding. The role will be developed to suit the skills of the successful appointee and support the needs of the practice.
Job responsibilities
* Take an active lead in prescribing activities and day‑to‑day medicine management, working closely with the prescribing clerk to ensure smooth and efficient prescribing.
* Undertake planned medication reviews with patients, both by telephone and face‑to‑face where appropriate, in line with the practice medication review policy.
* Conduct reviews in chronic disease management (e.g., hypertension, cardiovascular disease, respiratory disease) and other areas as experience permits.
* Participate in the practice prescription query process.
* Take part in medicine reconciliation, including review of hospital discharge summary letters and clinic letters.
* Participate in medicine management audits, including those suggested as part of the MMIS and other practice or health‑board defined projects.
* Engage in practice education and provide mentorship for the practice prescribing clerk, as well as supporting the wider clinical team in aspects of prescribing.
* Attend a weekly supervision hot‑review style session with a senior GP for ongoing clinical support and development of the new role.
Qualifications
* Registered Pharmacist with the GPhC.
* Strong knowledge of medicines management in primary care.
* Independent Prescriber status or willingness to undertake training if not yet qualified.
Experience
* Experience working in a patient‑facing clinical role.
* Excellent communication and consultation skills.
* Ability to work autonomously and as part of an MDT.
* Commitment to patient‑centred care and quality improvement.
* Previous experience in general practice or primary care.
* Experience developing or improving prescribing processes.
* Experience using GP clinical systems such as EMIS.
* Audit or service improvement experience.
