Permanent, Part Time (22.2 hours) £44,952 to £52,884 per annum (FTE) Location: Chelmsford Working Style: Anywhere worker Pro‑rated salary for 22.2 hours: £26,971 to £31,730 per annum.
About the Role
The Transforming Care Programme is part of a wider approach to Health Equalities for autistic children, young people and adults, as well as people with learning disabilities. The team works on behalf of the Essex ICB and three local authorities across Southend, Essex and Thurrock (SET) to transform health and social care systems and deliver improved health outcomes for people with learning disabilities and/or autism (LD/A).
The Case Manager role is essential in bridging strategic and operational elements of programme delivery, with a focus on the NHS Long Term Plan for those most at risk of institutionalised care. The role works within a hosted team to keep people safe in the community, prevent unnecessary admission to mental health beds, and facilitate timely discharges. It also supports preventative and integrated approaches for autistic individuals and/or those with learning disability who display challenging behaviours, ensuring community services are coordinated to avoid institutionalised care.
Responsibilities
* Provide specialist advice and professional expertise to health professionals and social workers.
* Demonstrate expert and effective practice in complex situations, supporting colleagues to mitigate high levels of risk.
* Make independent decisions and work across services (health, social care, education) to ensure timely and coordinated interventions.
* Chair Care and Treatment Reviews for relevant clients.
* Ensure barriers are removed and issues resolved through troubleshooting and stakeholder engagement.
Qualifications
* Degree level (Diploma or Degree in Social Work or Nursing) or equivalent.
* Evidence of continuing professional development and subject matter expertise in a relevant area; member of a relevant health or social care professional body.
* Significant experience working with people with learning disabilities and/or autism and their families, with an understanding of the functioning of health and social care systems and relevant legislation.
* Comprehensive, up‑to‑date understanding of relevant legislation, including the Mental Health Act.
* Experience chairing meetings with clients, families, carers and multi‑agency professionals and making recommendations on complex issues.
* Ability to challenge and negotiate with senior stakeholders on difficult issues to ensure services align with national policy.
* Excellent communication and interpersonal skills; ability to build and maintain relationships with health and social care professionals, individuals with learning disabilities and families.
* Experience identifying and implementing national policy related to learning disability and autism, researching best practice and applying it locally.
* Valid driving licence and access to a vehicle for travel around Essex and beyond.
