We are seeking a passionate and dedicated CSW - Social Worker - Disabilities (Gr 8)* to join our team in providing essential support and advocacy for individuals with disabilities. As a Social Worker specializing in disabilities, you will play a critical role in enhancing the quality of life for our clients and their families by connecting them with necessary resources and services. Your expertise will be essential in understanding the unique challenges faced by individuals with disabilities and implementing tailored support plans that promote independence and well-being. Responsibilities
* Conduct comprehensive assessments of clients' needs and strengths
* Develop and implement individualized service plans to address client goals
* Facilitate access to community resources, services, and support systems
* Provide ongoing case management and support to clients and their families
* Advocate for clients' rights and access to necessary services
* Collaborate with interdisciplinary teams to coordinate care and support
* Maintain accurate records and documentation in accordance with agency policies
* Participate in training and professional development opportunities to enhance skills
Requirements
* Master's degree in Social Work (MSW) from an accredited program
* Valid state social work license or eligibility for licensure
* Previous experience working with individuals with disabilities or in related fields
* Strong knowledge of disability services, resources, and advocacy
* Excellent communication and interpersonal skills
* Ability to work collaboratively with diverse populations and organizations
* Strong organizational skills and attention to detail
* Willingness to travel to meet with clients in various settings
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £27.68
Location: Birmingham, West Midlands
