 
        
        Senior Occupational Therapist - Discharge To Assess

Overview
Discharge to Assess will be a seven‑day service, requiring working some weekends and bank holidays on a rota basis. The post involves working across the main hospital site and the community, delivering holistic assessments and interventions to patients in their own homes or place of residence. Independent travel within the local geographical area is essential.
Responsibilities
 * Triag[e], assess, implement, plan and evaluate the delivery of direct patient care as an independent practitioner.
 * Facilitate timely discharge from an acute hospital or community bed and provide assessment and support planning to meet a person’s needs in the community.
 * Provide and arrange appropriate support and interventions in the community working closely with colleagues across all sectors.
 * Work flexibly between hospital and community settings, providing full Discharge to Assess service cover on an ad‑hoc or rotational basis.
 * Take a leadership role in Discharge to Assess and contribute to the delivery of high‑standard care, efficient use of resources, training, development and ongoing support.
Main Duties
The post holder will deliver specialist knowledge and clinical skills to ensure the delivery of care within the service and in the wider community and across the acute/community interface. They will contribute to effective budgetary and resource management, training and development and ongoing support.

Qualifications
 * Diploma/degree in Occupational Therapy
 * Registration with HPC
 * Post‑graduate courses
 * Fieldwork education
Knowledge/Skills
 * Knowledge of occupational therapy models of practice
 * Experience of supervising junior staff and students
 * Knowledge and application of assessment and interventions relevant to physical disabilities
 * Ability to analyse professional and ethical issues
 * Understanding of clinical governance and risk
 * Ability to reflect & critically appraise own performance
 * Ability to prioritise, delegate & organise
 * Ability to respond efficiently to complex information
 * Experience and ability to work as part of a broad multidisciplinary team across organisational boundaries
 * Experience of teaching junior staff and students
