Admissions, Transfers and Bed flow Co‑ordinator
Job responsibilities
* Flow Coordination and support using a Quality Improvement approach which is part of the Trusts strategic ambition.
* Responsibility for ongoing policy implementation & service development
* Provide expert knowledge of the London Compact agreement and Who pays guidance to ensure that only patients who are the responsibility of SABP are admitted to the wards.
* Providing the first response for escalations between trusts and having the skills, knowledge and communication skills to manage these professionally.
* Working closely with the MDT to ensure all people who are medically fit are reported correctly on Systmone and that these dates and the estimated discharge dates (EDDS) are tracked
* Undertakes sample checking to comply with service standards and audits.
* To assess, analyse, interpret and adjust information recorded on the Trusts main electronic patient record (EPR) systems for the purpose of ensuring accuracy of data throughout a patients pathway
* Interacting with clinical staff and supporting clinical staff in clinical areas with pathway management.
* Liaising with clinicians, reviewing patient health records and cross referencing with referral information to ensure safe transfers of patients
* Identifying overseas patients and reporting them correctly on Systmone and working with the overseas team to ensure safe repatriation
* Hold a budget to maintain flow by booking transport, temporary accommodation and flights
* Staff Management - This role involves direct line management of 1 Band 4 Administrator Manage, organise and support staff effectively through work planning, 1:1 meetings, appraisals and annual PDR including performance and productivity monitoring.
Qualifications
* Educated to degree level or level 5 in health-related topic or relevant experience.
Experience
* The post holder will have experience of working in a Mental Health or Acute setting at a Band 4 or above in a role which involves coordinating work
* Experience of clerical and administrative procedures and systems
* Experience of supervision would be an attribute, but training and development will be provided
