Join our growing Care Navigation Service as a Care Navigation Officer. This fast‑paced role requires a highly organised individual who can balance competing priorities and maintain accuracy while working to tight deadlines. You will use ICT systems and telephone communication to secure essential packages of care for residents across Lancashire.
Key Responsibilities
* Source care packages with external providers for residents using IT systems and complete all associated administrative tasks.
* Manage a workload of work independently, adhering to strict timescales and escalating cases where required.
* Communicate effectively with internal and external customers and colleagues at all levels via telephone, electronic and other media.
* Follow established processes, procedures and legal frameworks.
* Maintain accurate, up‑to‑date electronic records ensuring secure handling of data.
* Work as part of a busy team and collaborate with colleagues to meet service targets.
* Use creative thinking to solve problems with limited supervision.
* Develop excellent working relationships with both internal and external stakeholders.
* Cover miscellaneous tasks on rota such as allocating, handling mailbox queries, and distributing service information.
* Collaborate closely with managers and other professionals to deliver an effective, high‑quality service.
Further Information
* Full‑time permanent position based at the County Hall Complex.
* Service operates 364 days a year (closed Christmas Day); rota hours 8am‑6pm Monday to Friday, with weekend and bank holiday shifts 9am‑5pm.
