Specialist Community Nurse/District Nurse
Are you an experienced community nurse or District Nurse with leadership skills who aspires to provide holistic, patient centred care? Ryedale Community Team are based at Malton Community Hospital and serves the local community, and the wider rural population across the beautiful North Yorkshire countryside. You will co-lead a team of nurses in providing assessment & care to patients in their own place of residence. Because our service also provides nursing specialisms there is comprehensive support and advice available to you.
This is a fantastic opportunity to work within our integrated, multi-disciplinary Community Team. You will have well-developed clinical and leadership skills and work alongside our wider team including Therapists, Tissue Viability, Continence, Cardiac, diabetes, dietetics, Speech & Language and Respiratory Specialists.
Main duties of the job
Community Nursing is a highly specialised area of practice recognised by the Queens Nurses Institute and NMC as a specialist role.
As a District Nurse/Senior Community Nurse, you will work within an integrated hub model of District Nurses, Specialist Nurses, Community Staff Nurses, Therapists, Trainee/Registered Nursing Associates, Health Care Assistants and to meet the needs of the complex patient in the community and avoid admissions to the acute sector as far as practicable. This includes:
* Intravenous Antibiotic administration,
* End of life care,
* Managing the deteriorating patient,
* Complex and simple wound care including VAC therapy and compression therapy,
* Venepuncture,
* Utilising advanced physical assessment skills and many more clinical nursing skills.
Main duties of the role include:
* Provision of high quality, patient centred clinical care
* Caseload Management;
* Leading a team of Community Nurses and support staff including provision of supervision - both clinical & managerial;
* Working with other Band 6 District/Community Nurses to ensure adequate staffing and share resources and learning;
* Induction, probation & competency development of new staff recruited to team;
* Referral to and working alongside Specialist Clinicians & other agencies;
* Engagement with Safety Huddles;
* Actively encourage prevention and self-management;
* Carry out audits;
* Engage in working groups to inform service development.

Person Specification
Experience
* Evidence of development of specialist practice nursing skills and able to demonstrate the impact of this on practice change/development
* Evidence of promoting/supporting active user/carer involvement/participation
* Leadership/management experience which has had a positive impact and created change within the service delivery/practice
* community nurse experience
Qualifications and Knowledge
* Registered Adult Nurse on NMC live register
* Post registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice
* Demonstrable experience of mentoring pre-registration students
* Prepared to undertake or hold a recognised non-medical prescribing qualification, if require
* Leadership training/qualification/experience
Skills and Competencies
* Effective inter-personal skills and experience in supervising and mentoring
* Ability to effectively transport self between multiple addresses/venues on a daily basis including transporting equipment & clinical stock required for treatment
* Ability to lead and motivate staff to embrace change
